Image Type

Image Type

Click in the Image Type field, and then click on the option from the list that best describes the image.

If the image is normal and has information to be indexed, select Normal. Then press Tab or Enter.

If the image is blank, is an exact duplicate of another image, or is unreadable, select the corresponding option from the list. If the image contains only the back of a record card or otherwise has no data for the indexing fields, such as the name of the person who died, select No Extractable Data Image. Press Tab or Enter, and, on the warning message, click Yes. Continue to the next image. If this is the last image of the batch, submit the batch.

Certificate Number

Certificate Number

Type the certificate or state file number as it is written or stamped on the record. The number is usually located in the top center or top right corner of the page. If the digits in the certificate number have a space between them, don't include the space when you index this field.

The certificate number may also have been called the "state file number" on the certificate.

If a certificate number or state file number was not recorded, press Ctrl+B to mark this field blank.

Deceased's Given Names

Type the names that were written on the record in the First Name and Second Name fields of box 1.

If multiple deaths were recorded on one certificate, create a different record for each person.

Do not include titles or terms, such as Mr, Jr, or infant, with the name. If a name was abbreviated, type the name as it was written on the document. Do not type a period after the abbreviation. Do not index punctuation, except hyphens and apostrophes when they were written on the document as part of the name.

If given names were not recorded or were written as a variation of the word "unknown," press Ctrl+B to mark this field as blank.

Type the name that was written on the record in the Last Name of Deceased field of box 1. (On some forms, the field may be called "Last Named of Deceased.")

If a surname was not recorded, do not assume a surname from the surname of the father or mother.

Do not include titles or terms, such as Mr, Jr, or infant, with the name. Do not index punctuation, except hyphens and apostrophes when they were written on the document as part of the name. Do not expand abbreviations. Type what you see in the document.

Index the age that was written in box 8 of the form. If no age was recorded, do not calculate the age based on the dates of birth and of death.

Index only full years. If fractions of years were recorded, round down to the nearest full year. If less than one year was recorded or a child was stillborn, index the age as 0 (zero).

Type the name of the town or city where the individual died, as recorded in box 12a.

If the town or city was misspelled, spell it correctly. If it was abbreviated and you can determine what the abbreviation stands for, type the complete name instead of the abbreviation. If you are not sure what the abbreviation stands for, index what was written, excluding punctuation.
